Contactless Drop-Off Form Tax Preparation

WHAT IS IT?

Canada Revenue Agency has allowed the use of virtual tools to assist organizations in preparing income tax returns for their clients through the Community Volunteer Income Tax Program. To have your income tax returns filed by drop-off, you will fill out a Drop-Off Form with all of your information, and then send it to us. A CVITP volunteer will then review your information and prepare your taxes. We’ll give you a call if there is any additional information that we need. Once complete, a copy of your tax summary will be mailed to you.

WHO IS THIS FOR?

If you are eligible for having your taxes filed through the CVITP (find out if you are eligible by clicking HERE), can download and print the form, and can scan or take pictures of the completed form and documents to be uploaded to the website, then this is a great option for you!

HOW TO PREPARE

If you are married or in a common-law relationship, your partner must also fill out their portion of the form and include their ID and supporting documents so that your returns can be filed together, ensuring correct benefit calculations.

Make sure you (and your partner if applicable) know:

  • Your name as it is on file with Canada Revenue Agency

  • Your Social Insurance Number (SIN)

  • Your date of birth

  • If you have any children under 18 who live with you or infirm adults in your care, know their name, date of birth, SIN, and net income

  • Your sources of income, including any casual labour or tips that didn't generate an income slip

  • Eligible medical expenses (you can request a printout from your pharmacy!)

  • Charitable donations

  • Rent or property tax info

INSTRUCTIONS FOR FILLING OUT DROP-OFF FORM

These instructions are for filling out the above drop-off form for yourself / your family. If you are a service provider assisting your client by filling out this form, please go HERE for instructions on how to do that.

  1. Download the drop-off form from the link above. It will either open in a new window, or download directly to your downloads folder.

  2. Print off the form to fill out by hand OR complete the form on the computer.

  3. Complete all sections that apply to you.

  4. If you filled out the form on the computer, print it out now and sign.

  5. To verify your identity, either scan your ID and sign the photo, or take a selfie of yourself holding your photo ID.

You have three options for getting us the drop off form, signed authorization forms, and other supporting documents (ID verification, copies of rent receipts, etc).

Option 1 - Drop Off at our Office
Put your documents in an envelope, and clearly label the outside with your name and write the words “Client Drop Off” on the outside. Our office is located at 294 Portage Avenue (in the Somerset building across from the Dollarama) on the 5th floor. Suite 500 is directly in front of the elevators, and there is a mail slot and bin to the right of the door that you can put your clearly labelled envelope through.

Option 2 - Mail to the CFCS Office
Address your envelope as follows:

Option 3 - Secure Upload Through Our Website (e-Courier Security Deposit Box)

  1. Take a very clear picture or scan of each page of the drop off form and authorization forms.

  2. Finish reading these instructions, and then click the button below to open up the e-Courier Security Deposit Box. e-Courier is an end-to-end encrypted file transfer service so your data will kept absolutely secure. CRA has approved this process.

  3. Type in your Email.

  4. Upload those pictures or documents by clicking the button below and attaching them where it says Attachments.

  5. Put your last name, then first name followed by “Client Drop Off” in the Subject line. Example: SMITH, JOE - Client Drop Off

  6. Include any additional details about your tax situation or the files as needed in the Secure Message section.

  7. Double check your files were attached, then click Send Secure Package.

  8. Done!

Once we have received your drop off form and documents, if we need to contact you for more details, we will do so. Otherwise we will complete your taxes within 10 business days of receiving them and mail back to you your completed tax documents and any original documents (please send copies where possible as we are not responsible for anything lost in the mail) that were sent to us. Once your taxes are complete, we do not keep a copy of your documents, and all digital or physical copies are destroyed.
